Who Can Order Testing?

  • Domestic (U.S. & Canada) Clients
    Only healthcare providers licensed in their state may order laboratory testing. MDs and DOs are certified in all 50 states. If you are unsure of regulations in your state, please contact your governing or licensing board for assistance.
     
  • International Clients
    Only approved healthcare providers may order laboratory testing. If you are uncertain of regulations in your country, please contact your governing or licensing board (i.e. Ministry of Health) for assistance.

PATIENTS:  Please contact your medical practitioner regarding our tests. Metabolon does not quote test pricing directly to patients. Your practitioner must contact us directly.
 

Getting Started

You must have an account with Metabolon Precision Medicine for test orders to be processed.  

NEW CUSTOMERS – Please complete a New Client Form and submit to Precision Medicine Client Services by email or fax (919.287.2278).  

All orders must include a hard copy of the Patient Sample Manifest with sample shipment. You will receive a Patient Sample Manifest from Precision Medicine Client Services at the time you establish an account. 

Pricing Quotes
As a policy, Metabolon does not quote test prices directly to patients. Client Services is available to discuss pricing with a healthcare practitioner or his/her staff.

Billing & Payment
We do not accept patient insurance or provide professional/institutional billing services for reimbursement. The client is responsible for making full payment to Metabolon for any testing performed.

Payment options include credit card, wire transfer, and check (U.S. customers only).

The Patient Sample Manifest provides information we need to process your selected test. Required information includes:

Specimen collection, preparation and handling are essential steps in obtaining accurate test results. Proper procedures must be followed to assure the integrity of the specimen, as well as its safe and expedient delivery to the laboratory for processing. Metabolon does not provide specimen collection kits for tests.

Common Collection Errors

Careful attention to the specific requirements for each test specimen can help avoid common collection errors, including but not limited to:

  • Insufficient quantity
  • Over filling tubes with specimen that requires freezing
  • Blood collection and transfer errors, including the exposure of blood to extreme temperatures, could result in hemolysis
  • Failure to use the correct container for specimen collection
  • Inaccurate and incomplete patient guidance
  • Failure to label a specimen correctly
  • Failure to provide all pertinent information
  • Failure to tighten specimen container lids, resulting in leakage and/or contamination of specimen

 Reasons for Rejected Specimen

  • No Patient Sample Manifest included with sample shipment
  • Spoiled specimen
  • Submitted wrong specimen for test requested
  • Quantity Not Sufficient (QNS)
  • No collection date, inability to verify sample stability for test
  • Hemolyzed specimen

Common Causes of Aberrant Test Values

  • Medications: Sometimes prescription and over-the-counter medications can interfere with chemical determinations or alter levels of the substances being tested. Medications should be noted on the Patient Sample Manifest.

Shipping & Handling (Domestic Clients)

Please arrange delivery of specimens to Metabolon with a carrier who provides priority next-day delivery service. The client assumes full liability and responsibility for any costs related to the delivery of specimens to Metabolon. 

All orders must include a hard copy of the Patient Sample Manifest with sample shipment. Ship specimens Monday thru Thursday using priority next-day delivery to ensure delivery during normal weekday operations (Monday-Friday, 8:30 AM - 5:00 PM Eastern). For shipping around holidays, please contact us to confirm holiday scheduling.

Shipping & Handling (International Clients)

International clients must make shipping arrangements directly with a carrier who provides international priority delivery service. International shipments must be tendered under DDP incoterms, as the client assumes full liability and responsibility for all costs related to delivery of specimens to Metabolon. Please contact the carrier directly for questions and guidance regarding export/import requirements for shipping biological specimens. Please contact us with questions regarding Metabolon shipping policies.

International shipments must be accompanied by a Commercial Invoice that properly describes the package contents and customs value. For convenience, a Commercial Invoice Document is available for download. You are encouraged to use this document to avoid customs delays.

Commercial Invoice should be completed in triplicate (3x) and should not go inside the sealed package.

The printed copies should be viewable with the AirWay Bill on the exterior of the package.

GENERAL SHIPPING INFORMATION

The shipper is required to comply with the rules and guidelines for transport of biological specimens as set forth by the U.S. government, the government of the country of origin, and international regulatory agencies. Failure to follow instructions for packaging and shipping specimens may result in delay, loss or destruction of your specimens. Violation of regulations may be in breach of national laws and can be subject to civil or criminal penalties.

  • Shipper must properly identify, classify, package, mark, label and document contents of shipment in accordance with the laws and regulations of the countries of origin, transit and destination.
  • Shipper must notify Metabolon of the shipping details for tracking purposes.
  • All samples received by Metabolon must be properly identified. Clearly label each specimen with a permanent marker including three identifiers:  patient sample identifier (required) plus two others (i.e. patient name, date of birth, or collection date). The information used to label the specimen must also be provided on the Patient Sample Manifest.
  • Specimen integrity is essential. The tops on all tubes should be securely sealed prior to shipping to help assure the safe arrival of specimens and prevent testing delays. Properly position vials so that they are secure in the secondary packaging and covered both top and bottom with dry ice.
  • A printed, completed Patient Sample Manifest must accompany the shipment, or the test cannot be processed. Precision Medicine Client Services will provide a manifest form to you when you become a registered client. Be sure to follow the specific test collection and shipping preparation instructions for appropriate handling of specimens (refer to Specimen Requirements to download test-specific instructions).
